First stop + verify — what is the main presentation?
💡 For all suspected reactions, step one: stop the transfusion immediately, keep the line open with normal saline, verify identity/blood bag, notify the blood bank and return the bag for testing (recheck type/cross-match/DAT). Then triage by presentation: fever ± hemolysis / respiratory distress / fever or rash only.
