Adnexal cystic structure — tubular shape vs ovarian origin?
💡 Hydrosalpinx = distal obstruction causing tubal fluid (pus = pyosalpinx, blood = hematosalpinx); usually secondary to PID, ligation, endometriosis, postoperative adhesions. US signs: a tubular/S-C-shaped cyst separate from the ovary; cogwheel sign (thickened folds on cross-section = spokes, subacute), beads-on-a-string sign (mural nodules 2–3 mm, chronic), waist sign (opposing-wall indentations, + tubular = specific), incomplete septa; no flow.
