🧫 Mehran Score (Contrast-Induced Nephropathy)

Predict contrast-induced nephropathy risk after PCI with the Mehran score.

Mehran Score (Contrast-Induced Nephropathy)

Hypotension
Intra-aortic balloon pump
Congestive heart failure
Age > 75
Anaemia
Diabetes
Contrast volume (mL)
eGFR

When to use

Identify patients needing renal protection around contrast procedures.

How it works

Hypotension 5, IABP 5, CHF 5, age > 75 4, anaemia 3, diabetes 3, contrast 1/100 mL, eGFR 40–60 (2)/20–40 (4)/<20 (6). ≤5 low to ≥16 very high.

Key points

  • Risk rises steeply across strata (≈7.5% to ≈57%).
  • Minimise contrast volume and ensure hydration.
  • Hold nephrotoxins and monitor renal function.
