Full pathway

1. [Decision] Severity + response to conservative care
   Severity (denervation/thenar atrophy? EMG) + response to conservative care?
   • Mild-moderate (intermittent numbness, no atrophy/denervation) → Mild-moderate → conservative
   • Severe (persistent numbness, thenar atrophy/weakness, EMG denervation) or conservative failure → Severe/failed → carpal tunnel release
   • Acute (acute carpal tunnel after fracture/hematoma) → Acute compression → emergency decompression
2. [End] Mild-moderate → conservative
   Mild-moderate → conservative: neutral nighttime splint, activity modification, carpal tunnel corticosteroid injection (short-course oral steroid is also an option); re-evaluate for surgery if ineffective.
3. [End] Severe/failed → carpal tunnel release
   Severe (thenar atrophy/persistent numbness/EMG denervation) or conservative failure → carpal tunnel release (open or endoscopic; endoscopic recovers faster, open is the standard).
4. [End] Acute compression → emergency decompression
   Acute carpal tunnel (acute median nerve compression after fracture/hematoma/infection) → emergency decompression and release.
