심장 독성이나 QRS 확대가 있습니까?

💡 삼환계(아미트리프틸린 등)는 나트륨 통로를 차단해 QRS를 넓힌다(>100 ms는 경련을, >160 ms는 심실성 부정맥을 예고한다). aVR에서 R′가 나타난다. 항콜린 증상(동공 산대, 빈맥, 건조하고 붉은 피부, 섬망), α 차단에 의한 저혈압, 경련, 혼수가 동반된다. 심전도와 임상 소견으로 판단하며 혈중 농도의 의의는 제한적이다.
