동적 골반저 MRI(HMO 체계, 최대 힘주기·배변 상) 소견은?

💡 동적 골반저 MRI(MR 배변조영)로 세 구획을 동시에 평가한다(전방 = 방광·요도, 중앙 = 자궁·질, 후방 = 직장·항문). 직장(± 질)에 젤을 주입하고 시상면 SSFP로 안정·항문 조임·힘주기·배변 상을 촬영한다(배변 상이 발살바 단독보다 탈출 검출에 우수하다). 기준선: PCL(치미선: 치골결합 하단 → 말단 미골), H선(치골결합 하단 → 항문직장 접합부, 정상 약 5 cm), M선(PCL에서 H선 후단까지의 수직 거리, 정상 약 2 cm). 탈출 = 힘주기 시 장기가 PCL 아래로 내려가는 것.
