안와격막 전방입니까, 후방입니까?(안구 운동통·안구 돌출·안구운동 마비)

💡 안와격막 후방의 연조직(근육·지방) 감염이다(전방은 눈꺼풀에 국한된다). 소아에 많고 86–98%가 부비동염(사골동)에서 기원하며 치성·외상성도 있다. 감별 요점: 안구 운동통, 안구 돌출, 안구운동 마비, 구결막 부종 ± 시력 저하·RAPD와 함께 발열과 백혈구 증가가 나타난다(눈꺼풀의 발적·부종은 양쪽 모두에 있어 감별이 되지 않는다). 양측이면 → 해면정맥동 혈전을 의심한다. 합병증: 시력 상실(약 11%), 안와 골막하 또는 안와 농양, 해면정맥동 혈전, 수막염, 뇌 농양.
