고전압, 심전도 이상, 부정맥 또는 미오글로빈뇨가 있습니까?

💡 전류가 조직을 통과하면서 부정맥(심실세동·심실빈맥·무수축), 신경 손상(의식 소실·무호흡·경련), 근육 강직에 의한 골절·탈구, 심부 조직 괴사(대개 피부 소견보다 심하다), 혈관 색전과 부종에 의한 구획증후군, 횡문근분해 → 미오글로빈뇨 → AKI가 발생한다. 고전압(≥1000 V)은 중하고 낙뢰는 대개 표재성이다. 평가: 모든 환자에게 심전도를 시행하고 CK·요 미오글로빈·신기능·트로포닌을 검사하며 구획증후군과 동반 외상을 확인한다.
