When to use

How it works
Energy 25–30 kcal/kg; protein 0.8–1.5 (stable) / 1.2–2.0 (critically ill) / 1.5–2.0 (burns) g/kg; fluid 25–40 mL/kg; lipid ≤ 1 g/kg.
Key points
- Caloric factors: protein 4, glucose 3.4, lipid ~9–10 kcal/g.
- Peripheral PN: glucose ≤ 10–12.5%, osmolality < 900 mOsm/L, ≤ 7–10 days.
- Restrict lipid if triglycerides > 400; monitor glucose, electrolytes, LFTs.
- Start at ≤ 50% energy in refeeding-risk patients and replace electrolytes/thiamine first.
