🫀 Milan Criteria (HCC Transplant)
Check whether hepatocellular carcinoma meets the Milan criteria, the classic morphologic standard for liver transplant candidacy.
Milan Criteria (HCC Transplant)
Number of tumors (lesions)
Largest tumor diameter (cm)
Macrovascular invasion
Extrahepatic spread
When to use
Assess transplant eligibility in HCC based on tumor burden and spread.
How it works
Within Milan = single lesion ≤ 5 cm, or ≤ 3 lesions each ≤ 3 cm, with no macrovascular invasion and no extrahepatic spread.
Key points
- Within-criteria transplant gives ~ 70% 5-year survival.
- Outside-criteria patients may qualify via expanded criteria (UCSF, up-to-7) or after downstaging.
- Macrovascular invasion or extrahepatic spread is usually a contraindication.
