When to use
Classify liver injury as hepatocellular, cholestatic or mixed to direct further testing.
How it works
R = (ALT/ULN) / (ALP/ULN): hepatocellular > 5, cholestatic < 2, mixed 2–5.
Key points
- Albumin and INR reflect hepatic synthetic reserve.
- Conjugated bilirubin > 20% of total suggests hepatobiliary disease.
- Isolated ALP rise: check GGT to separate hepatic from bony source.
- Isolated indirect bilirubin is usually Gilbert syndrome.
