When to use

Confirm haemolysis, then classify by DAT (immune vs non-immune) and the intravascular/extravascular pattern to find the cause.

How it works

Haemolysis: LDH ↑ + haptoglobin ↓ + indirect bilirubin ↑ + reticulocytes ↑.

Key points

  • LDH ↑ with haptoglobin ↓ is sensitive and specific for haemolysis.
  • DAT separates immune (AIHA) from non-immune haemolysis.
  • Schistocytes flag microangiopathic haemolysis (MAHA).
  • MAHA needs urgent workup for TTP/HUS/DIC.
