💊 DAPT Score (Dual Antiplatelet Duration)
Weigh prolonged dual antiplatelet therapy after PCI with the DAPT score.
DAPT Score (Dual Antiplatelet Duration)
Age
Current cigarette smoker
Diabetes mellitus
MI at presentation
Prior PCI or prior MI
Stent diameter < 3 mm
Paclitaxel-eluting stent
CHF or LVEF < 30%
Saphenous vein graft PCI
When to use
Decide whether to extend DAPT beyond 12 months.
How it works
Age ≥75 (−2)/65–74 (−1); smoking, diabetes, MI at presentation, prior PCI/MI, stent < 3 mm, paclitaxel stent each +1; CHF/LVEF < 30% and vein-graft PCI +2. ≥ 2 favours prolonged DAPT.
Key points
- Applies to patients event-free and DAPT-tolerant at 12 months.
- ≥ 2 → ischaemic benefit likely exceeds bleeding harm.
- Combine with a formal bleeding risk assessment.
