3. In the past 4 weeks, how often did asthma symptoms (wheeze, cough, breathlessness, chest tightness) wake you at night or earlier than usual?
4. In the past 4 weeks, how often have you used your rescue inhaler (e.g. salbutamol)?
5. In the past 4 weeks, how would you rate your asthma control?
