How is the result calculated?

MELD = 3.78·ln(bilirubin) + 11.2·ln(INR) + 9.57·ln(creatinine) + 6.43; labs floored at 1.0, creatinine capped at 4.0 (or with dialysis). MELD-Na adds a sodium term when MELD > 11.

Is this a substitute for clinical judgement?
