1. When you point at something across the room, your child usually does NOT look at it
2. You have wondered whether your child might have a hearing problem
3. Your child rarely plays pretend or make-believe (e.g. pretend to drink, use a block as a phone)
4. Your child does NOT like climbing on things (furniture, stairs, slides)
5. Your child often makes unusual finger movements near their eyes (e.g. wiggling fingers close to the eyes)
6. Your child does NOT point with one finger to ask for something out of reach
7. Your child does NOT point to show you something interesting
8. Your child is NOT interested in other children (does not look at, approach, or play with them)
9. Your child does NOT bring objects over to show you (just to share, not for help)
10. Your child usually does NOT respond when you call their name
11. When you smile at your child, they rarely smile back
12. Everyday sounds (vacuum cleaner, music) upset your child a lot
13. Your child does NOT yet walk
14. Your child rarely looks you in the eye when you talk to, play with, or hold them
15. Your child does NOT imitate your movements (e.g. wave bye-bye, make a face)
16. When you turn to look at something, your child does NOT look in the same direction
17. Your child does NOT try to get you to watch or notice them (e.g. say 'look at me', do something for attention)
18. Your child does NOT understand simple instructions (e.g. 'put the book on the table', 'bring me your shoe')
19. When something new happens, your child does NOT look at your face to gauge how to react
20. Your child does NOT like movement activities (being swung, bounced or jiggled)
