Ischemic or non-ischemic (blood gas/ultrasound)?

💡 Erection >4 h unrelated to sexual activity. Ischemic (low-flow/veno-occlusive, 98%, a penile compartment syndrome, painful + rigid corpora with a soft glans, hypoxic/hypercapnic/acidotic blood gas) = a true emergency; non-ischemic (high-flow/arterial, usually traumatic, painless, normal blood gas). Diagnosis: corporal blood gas + Doppler ultrasound to distinguish (ultrasound before aspiration). Time-sensitive: >24 h ~90% ED, >48 h near-irreversible fibrosis.
