4T score (thrombocytopenia/timing/thrombosis/other cause)?

💡 HIT: anti-PF4-heparin antibodies activate platelets → thrombocytopenia + paradoxical thrombosis (venous > arterial, not bleeding). Each 4T item 0–2: degree of platelet fall (>50% with nadir ≥20 = 2), timing (5–10 days after exposure or recent exposure <1 day = 2), thrombosis (new = 2), other cause (none = 2). Total 0–3 low, 4–5 intermediate, 6–8 high.
